All of our Camper Forms must be accessed through your online account. Every camper, once enrolled, has an online account, whether or not you enrolled online. To access your online account, click the 'My Account' link just to the left. After you enter your login information, click on 'Forms and Documents' under the Forms Dashboard and there you will see which forms are needed for your camper and when they are due.
Some of our forms can be completed entirely online (Camper Personal History, Health History & Transportation) and some forms will need to be printed out, completed, and faxed in to our office (Doctor's Form, Parent's Consent and Insurance, and sometimes the Immunization History). If you have returning campers the online Camper Personal History and Health History will be saved from last summer; these forms will only need to be reviewed and updated. The Doctor's Form and Parent's Consent and Insurance Form must be completed new every year your camper returns to Mountain Camp.
